End of Life Care at Home
End of life care can be provided in a person’s home, and there are many services that we can offer to ensure that they are comfortable and safe. First and foremost, we feel it is important to provide compassion and support to someone who is coming to the end of their life. This may involve simply being present and offering a listening ear, or it could involve more practical assistance such as:
- Helping patients manage their pain with the help of appropriate pain killers and other pain-relieving techniques
- Applying dressings to wounds
- Changing catheters and other medical equipment that the patient requires
- Ensuring that all devices and equipment are correctly cleaned and stored
- Providing personal care such as help with washing, showering, toileting, dressing, eating and oral care
- Monitoring the patient’s condition and informing the other health care practitioners who may also be involved in their care
